Section 11.2.1 says: 

   An
   example of this calculation is shown by the rootdist() routine in
   Appendix A.5.1.1.

It should say:

   An
   example of this calculation is shown by the root_dist() routine in
   Appendix A.5.5.2.

Notes:

No rootdist() routine is in Appendix. root_dist() appears in Appendix A.5.5.2.
