Section 3.6 says:

One example of such a form is the Incident Reporting Form provided by
   the CERT Coordination Center:

It should say:

Notes:

The URL with an example of an Incident Reporting Form is wrong. The hostname 'info.cert.org' no longer resolves. CERT.be still has a copy of a sample Incident Reporting Form.
