Section 4 says:

     revision 2016-04-09 {

     }

It should say:

     revision 2016-06-21 {
       description
         "Initial revision.";
       reference
         "RFC 7895: YANG Module Library";
     }

Notes:

Initial revision of ietf-yang-library YANG module was 2016-06-21, not 2016-04-09.

This is a valid issue but fixing it requires a new version of the YANG module to be published, which cannot be done via the errata process, hence resolved as "Held for Document Update".
