Minor textual improvements:

a) Change the first line of section 6.1., on page 6, from

  "The intent of these headers are to record telephone number that is
   sent by the analog phone system with an incoming call without
   alteration or interpretation." ...

to:

b) In the first line of section '10. Acknowledgments', on page 9,
change:

  "The previous authors of versions of this document were Derrick Dunne

to:

   and Jason Collins." ...

It should say:
