Section 3 says:

   rate-map-table  = 8rate-map-entry

It should say:

   rate-map-table  = *rate-map-entry

Notes:

I believe this is a simple typo specifying that there are num-rates number of rate-map-entries

Yes. Something like n-rate-map-entries would be even clearer (* implies a pointer to me)
